The CLINIC Registeration FORM  will be in your clinic package and must be
submitted to the registrar after each clinic,within 5 days
Please make sure you have  completed all information before you send the
form to the register.
Send your registration and clinic money to the Treasurer. No Cash in the mail.
Deposit your money and send a cheque.
Registration fees for 2009 season
Level # 1        $50:00 plus $10.00 clinic fee        Total=$60.00
Level # 2        $50:00 plus $10.00 clinic fee        Total=$60.00
Level # 3        $65:00 plus $20.00 clinic fee        Total=$85.00
Level # 4-5     $70:00 plus $20.00 plus national certification fee $50.00  Total= $135:00
If you have a Excel, you can send the report by Excel, it does
make the registrar life a lot easier and there are less mistakes.
You still must send in the written report to the Treasurer,with
your money..
